Get an Electronic or Paper Copy of Your Medical Record

You can ask to see or get an electronic or paper copy of your medical record and other health information we have about you. We will provide a copy or summary within 30 days of your request. A reasonable, cost-based fee may apply.

Ask Us to Correct Your Medical Record

You may request correction of health information you believe is incorrect or incomplete. We may deny the request but will provide a written explanation within 60 days.

Request Confidential Communications

You can request contact through specific methods or addresses. We will agree to all reasonable requests.

Ask Us to Limit What We Use or Share

You may ask us not to use or share certain health information. We are not required to agree unless you paid out-of-pocket in full and request restriction for insurance purposes.

Get a List of Disclosures

You may request an accounting of disclosures for the past six years. One request per year is free; additional requests may incur a reasonable fee.

Choose Someone to Act for You

A person with medical power of attorney or legal guardianship may exercise your rights on your behalf. We will verify their authority before taking action.

File a Complaint

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you.
